Building a strong foundation... The CHC Foundation is the hospital's fundraising arm. While it is a separate 501(c)(3) charitable, nonprofit, membership corporation, its sole member is the hospital. The CHC Board of Trustees appoints the Foundations' Board members, and a trustee along with the hospital CEO are designated positions on the Foundation Board to assure contributors that all donations are directed towards equipment and programs to benefit our patients. How can you help? Every day many lives are changed here at CHC. For some, it's the birth of their first child or grandchild. For some, it's the death of a loved one. For others it's a diagnosis of a serious disease or, hopefully, the news of good health. Isn't it reassuring that these significant events, whether joyful or sorrowful, can be shared close to home among family and friends? Your gift helps guarantee CHC will be here to continue to diagnose, heal, comfort, assist and aid our friends and neighbors throughout their lives. Gift Acknowledgment All gifts, regardless of value, form, or designated use, shall be acknowledged by official correspondence or receipt from the CHC Foundation. These documents represent to the donor CHC Foundation's offical acceptance of this gift and its terms and conditions as well as official certification of their chartiable act for tax purposes. Your gift can be directed to one of three types of funds: 1.DESIGNATED - Donations to these funds will be used as designated by the donors, eg. Hospice, Oncology, Pediatrics, etc. 2.UNDESIGNATED - Donations to this fund may be used for any reason consistent with the Community Health Center's and CHC Foundation's Mission and Vision Statements. 3.ENDOWMENT - Donations to these funds are restricted endowment funds in which only the interest on the principle may be used each year as designated by the donors. The Tree of Life and the donor recognition wall in the hospital's main lobby are utilized to recognize Patron donors and above. Special recognition is designated by the CHC Foundation Board of Directors for Founder's Society members. A donor's right to privacy and request for their gift to remain confidential will be respected. Only gifts of $250 or more per year will be cumulative beginning January 1, 2000. Donor's contribution levels will be moved up with additions during the year of contribution. Commemorative Gifts Commemorative gifts are welcomed. The honoree will be mailed an announcement of the gift unless the donor indicates otherwise. Memorial announcements will be mailed to the family of the deceased. The amount of the gift will not be divulged unless so requested by the donor.
